INTAKE & REFERRAL PROCESS MADE EASY

From answering your questions to providing transportation to treatment, Seafield is here to help make the intake process as easy as possible. Contact us anytime.

In some cases, a patient's insurance may require pre-certifications prior to admission. We provide assessments & pre-certifications at any of our Seafield outpatient facilities.

FOR PATIENTS:

Please bring the following:

Health insurance card
Prescription card & 30 day supply of daily medications.
Photo ID
A week’s worth of appropriate clothing
Money for miscellaneous expenses ($20/$30) for laundry, vending machines, phone calls & detergent.

ADDITIONAL THINGS TO KNOW:

Clients under the age of 18 must be transported to & from Seafield by their parents or legal guardian & must bring their immunization record.
Electronic devices such as laptops, radios, TV, cell phones, CD players, and iPods, are not allowed.
Mail can be sent, so please bring stationery & postage stamps.
Pay phones are available (at the discretion of the client’s Case Manager) at assigned times.
Vitamins or food items will not be allowed. The medical department will supply vitamins as needed.

If you are an OASAS licensed outpatient program, EAP and/or union & determine that a patient meets the inpatient criteria, call us for direct admission.
